Pre-Surgical Mental Health Evaluations for Bariatric Surgery Patients
At Deconstruction Counseling, I provide bariatric surgery psychological evaluations for adults in Kansas who need a mental health assessment as part of their pre-surgical requirements. These evaluations are designed to help assess emotional readiness, mental health history, eating patterns, support systems, coping skills, and your ability to follow post-surgical recommendations.
This is not about “passing” or “failing.” The goal is to provide your surgical team with helpful clinical information while also helping you feel more prepared for the emotional and lifestyle changes that can come with bariatric surgery.
Cost and Insurance Information
The fee for a bariatric surgery psychological evaluation is $300 and includes a 100-minute (1 hour 40 minute) appointment along with the time required to review clinical information, complete documentation, and prepare your written evaluation report for your surgical team.
Because this service is considered a specialized evaluation rather than ongoing psychotherapy, bariatric surgery psychological evaluations are not covered by insurance and are private pay only. Payment is due at the time of service unless other arrangements have been made in advance.
Many clients appreciate the transparency of a flat fee, which allows you to know the full cost upfront with no surprise billing. If your bariatric surgery program requires additional paperwork or forms beyond the standard evaluation report, please discuss this prior to scheduling.

I am a master’s-level clinician and a Licensed Professional Counselor under supervision for clinical licensure in Kansas. Bariatric surgery psychological evaluations are completed within my clinical scope and may involve consultation or supervision as appropriate.
Before scheduling, please confirm with your bariatric surgery program that they accept evaluations completed by a master’s-level LPC under supervision. Some programs may specifically require a psychologist, psychiatrist, or independently licensed provider.
